Acces failed

i'm getting acces failed , and i added the fire.dll to exclusions at my antivirus

I solved that by getting the november patch but now i gen an error as soon as i see the server choosing screen , looking for an answer please tell me how can i fix that

"l2.exe has encountered a problem and needs to close . we are sorry for the inconvenience . " bla bla ..
